Once again this year Bentonville Sports Medicine and Northwest Medical are partnering to provide free sports physicals to all athletes participating in a Bentonville Public Schools sponsored sports. This year physicals will administered locally at the Northwest Medical Clinic located off New Hope Rd. in Rogers. Again, these physicals will be free of charge on a walk-in basis during the times listed below.

To help expedite the process, Northwest Medical will be providing additional staff to assist Bentonville families during these times. In the event that you are unable to make your scheduled date and time we have provided “OPEN” dates in order to accommodate parents’ schedules. Bentonville Sports Medicine will not make physical appointments for athletes.

This year we are requesting that a copy of your child’s physical be given to the coach of each sport they are participating in. For example, if your child participates in football, basketball, and track, then they need to give a copy to each coach for that sport. We also recommend that parents keep a copy of the physical for themselves, in the event a form is misplaced. If you have any further questions please feel free to contact us. I will be sending this email out weekly until the end of May as a reminder.
